Case: Mild Perilymphatic Nodularity in Sarcoidosis

So here's a classic case of sarcoidosis.

I'm sure most of you who have experience with chest imaging,

you guys probably looked at it and said, oh, okay.

I know exactly what this is. And so what are we seeing here?

We're seeing these clustered nodules.

So perilymphatic nodules, especially in the setting

of sarcoidosis, it likes to do this

where it involves certain portion

of lungs on the axial slice pretty severely.

And other portion of lungs, like not at all

as we can see there, but what tells us is paralympic.

Well, we see the subpleural nodularity.

So all this subpleural nodularity,

you're already pushing central lobular nodularity, very,

very low on your differential diagnosis.

And also this fi nodularity, which really just a subtype

of subpleural nodularity.

And if you look carefully, we see a lot of these nodular

and para bronchovascular tree

and some along the interocular septa like this.

And again, the fact that it's not entropic,

it clearly has a form to them.

They, they want to involve certain portion of the lungs

as opposed to random nodule,

which they're just gonna go everywhere and anywhere.

Um, this really draws us more

toward a perilymphatic pattern.

And this patient has underlying history of sarcoidosis.

And so I could carry the mi, but I'm not going to.

And then for those of you guys who like to look

for lymph nodes, we can look at this so clearly,

bilateral symmetric, calcified lymph nodes within the parit,

tracheal, SubCal,

1:22

and hilar regions, typical of sarcoidosis.
