So here's an example on the left of Paraseptal emphysema.

These are pretty obvious cases of emphysema,

but I'm just showing these to you to remind you that

remember, emphysema is so,

so much more common than diffuse cystic lung disease.

I don't know what order, but I'd say it's probably like a

hundred times more common, maybe even more than that.

And so sometimes you get emphysema,

which is a little atypical, or you get emphysema

with some sort of superimposed acute process on it

and it can look funny and mimic diffuse cystic lung disease.

So when you think about diffuse cystic lung disease

first, slow down.

Take a few steps back

and ask yourself, is this just a case of emphysema?

Like if, if the patient's coming in acutely,

make sure you look, try to look at some older CT scans

as well to make sure the patient just didn't have

underlying emphysema.

In these cases like this where it's confluent emphysema,

I've seen some people sometimes mistaking this

for diffuse cystic lung disease.

Buting. In these cases, actually more cut and dry.

Clearly this is just a case of confluent

advanced upper lung preponderant emphysema.
