So here's example of beautiful central lobular

nodularity of ground-glass attenuation

all throughout the lungs in a patient

with non fibrotic hypersensitivity pneumonitis,

no fibrosis here but, but look at this beautiful example.

These fine central lo grl nodules, they have an order

to them as you could see.

And so this is pretty typical of non fibrotic hp.

And when you see this, this degree of ground lass, opac

and inflammation within the lungs, the air trapping

that we commonly see is either not present in cases

of HP like this

or the for some reason are concentrate the lung base.

So you see at the lung base there's a little

bit of mosaic attenuation.

Let's go ahead and pull up the coronal reformation

'cause I think the contrast is quite helpful.

See, I see the coronal reformation shows the basler

mosaic attenuation, which are areas of air trapping,

but then the rest of the lungs, it's all inflamed lung.

These diffuse central lobular long lasts nodules.

And so if we really mag it up, you'll see

that there's relative subpleural sparing.

So classic for central lobular nodularity in this patient
