Spondyloarthropathies In the Spine

<v ->Well, let's move on now

0:02

and let's talk a bit about the spine and what occurs

0:06

in the spondylarthropathy.

0:10

To understand this you'd have to know the anatomy.

0:14

So here's my drawing

0:16

of a typical discal vertebral junction.

0:19

This is anterior, this is posterior,

0:22

so this is a sagittal drawing.

0:25

Each vertebral body has an elevated rim,

0:28

you can see that here, right, with a central depression.

0:33

The central depression is covered by

0:35

a cartilaginous endplate.

0:38

Each intervertebral disc has a somewhat central,

0:41

not perfectly central, nucleus pulpolsus

0:45

surrounded by concentric fibers

0:47

that we designate the annulus fibrosis.

0:50

As you move to the very anterior and very lateral aspects

0:55

of the annuluus fibrosis,

0:57

you will find strong fibers

0:59

that are known as Sharpey's Fibers

1:03

that are a strong site of attachment of the disc

1:07

to the vertebral bodies.

1:10

And I've drawn these carefully here to point out

1:12

that those fibers, those Sharpey's Fibers,

1:16

attach not at the corner of the vertebral body,

1:20

but several millimeters from that corner.

1:23

So, I show you with these black arrows

1:25

and this sagittal section, this specimen,

1:29

the approximate location of the attachment

1:32

of Sharpey's Fibers.

1:34

Now that's gonna become important

1:36

in the pathogenesis of one type of outgrowth

1:39

that occurs in the spine.

1:41

Running anteriorly, the anterior longitudinal ligament

1:44

and posteriorly, the posterior longitudinal ligament,

1:47

so that is the pertinent anatomy that you need to know.
