Although we can see this in rheumatoid,

the more extensive chest wall involvement

occurs in the spondyloarthropathy

and that includes the sternoclavicular joint

and the sternomanubrial joint as well.

And what you're gonna see

is irregularity of the opposing bone surfaces,

bone proliferation,

and extensive marrow edema

as in this particular case.

Now there are other conditions

these two cases I'm going to show you.

There are other conditions that involve the chest wall.

And one of the very interesting ones,

and I'll take a moment to describe it is this.

Boy, that's a long name.

Sternocostoclavicular Hyperostosis, often abbreviated SCCH.

It's seen in older people.

It's a condition associated with bone proliferation

involving the medial ends of the clavicles,

the adjacent sternum, the upper ribs,

and the intervening soft tissue.

So you'll see this

bizarre bone proliferation at those sites,

and you can see sternomanubrial involvement as well.

I can tell you when you see this disease

on a chest radiograph,

you'll make the mistake that I made and call it lymphoma,

thinking you're dealing with mediastinal adenopathy,

but you're dealing with ossification about this chest wall.

And in some of those cases,

you'll end up with bone proliferation in the spine

resembling either ankylosing spondylitis,

or even DISH.

So that's an example of this condition.

Now what's interesting about it,

if you talk to the patients,

many of them will tell you

that they had pustular skin lesions

on their hands and feet.

The exact diagnosis

pustulosis palmaris et plantaris.

And it's the association of pustular skin lesions

with bone proliferation.

You see it in pustular psoriasis as well.

That is carried forth in this diagnosis.

We have a fancy name for that association.

We call it SAPHO syndrome,

S-A-P-H-O.

Synovitis, acne, pustulosis, hyperostosis, osteitis.

This is one of the conditions that falls

under this particular name SAPHO.

There are others, you may know them.

For example,

chronic recurrent multifocal osteomyelitis is one of them.

But in any case,

this is one of the conditions tying together

with involvement of bone,

particularly the chest wall.
