Psoriatic Onycho-pachydermo-periostitis

0:00

<v ->Now there is an entity that really, in my view,

0:03

looks like just an extreme dactylitis in psoriasis,

0:09

but it goes by a fancy name of the POPP Syndrome,

0:13

psoriatic onycho-pachydermo-periositis.

0:17

Some people say periostosis.

0:20

But in any case, it is not common.

0:23

And it was initially described as involved in the great toe,

0:26

but I think you can see it elsewhere not just in the foot,

0:30

but in the hand.

0:31

And it consists really of several abnormalities combined,

0:35

including osteoitis, right,

0:38

nail abnormalities in that same region,

0:41

soft tissue thickening so you end up with a macro toe,

0:45

particularly in the distal aspect as shown here.

0:49

And once again, what you will see is marrow edema

0:54

predicting that bone proliferation has occurred.

0:57

There is a differential diagnosis.

0:59

I look at this list

1:01

and I promise you you're gonna see some faces of gout

1:04

and even BPOP during this course,

1:07

just not at this moment.
