<v ->So, enthesitis, again, inflammation

0:03

where a tendon, a ligament,

0:05

or a joint capsule attaches to bone.

0:08

Characteristic of the spondyloarthropathies.

0:11

This is a beautiful case sent to me

0:13

by one of our previous clinical fellows,

0:16

and all the arrows are pointing

0:18

to areas of bone proliferation in multiple bones.

0:22

Femur and tibia, patella, tibial tuberosity

0:27

and a lot of these, not all of them,

0:29

relate to capsulitis, all right,

0:31

where the joint capsule attaches.

0:33

Not all, but this whiskering,

0:35

this irregular bone proliferation,

0:39

often associated with adjacent soft tissue edema.

0:42

This is absolutely characteristic of enthesitis.

0:46

And although you may see some in rheumatoid,

0:49

you don't see much.

0:52

So when you see this particular finding,

0:55

enthesitis, think spa, especially psoriasis.

1:00

Now look at this example, beautiful.

1:03

You're looking at enthesitis, with bone proliferation,

1:07

mainly involving an extrinsic ligament of the wrist.

1:10

This is a radioscaphoid ligament,

1:13

portion of the radial-collateral ligament of the wrist.

1:16

The main proliferation is at its radius

1:19

and scaphoid attachment sites.

1:21

Extensive edema associated with it,

1:24

and you can see here on even the radiograph,

1:27

the bone proliferation.

1:29

This is enthesitis.

1:31

Again, you won't see that in rheumatoid.
