And I'll just briefly tell you what the classic finding is,

Bilateral symmetrical involvement,

with diffuse loss of joint space in the hips

and a ring of osteophytes that grows

across the head-neck junction.

That's a characteristic, probably diagnostic,

of ankylosing spondylitis.

Here we see involvement of the trochanter.

Again, this is enthesitis in patient

And even at the heel, superior aspect

of the calcaneus in the area, the retrocalcaneal bursa,

the plantar aspect of the bone,

near or at the attachment of portions of the plantar fascia.

Now in rheumatoid, you might see something like this,

but on the plantar aspect of the calcaneus and rheumatoid,

you generally see little or nothing.

So if you have extensive bone proliferation

on the plantar surface,

particularly associated with marrow edema, you wanna say,

"Gee, this is probably a spondylo arthropathy."

enhancement in those same two areas,

retrocalcaneal bursitis, Achilles tendinosis,

some would argue maybe some low grade interstitial tearing,

plantar fascial changes, plantar calcaneal enthesophyte,

that's edematous, and edema within the calcaneus itself.
