Aseptic Lymphocytic Vasculitis-Associated Lesion (ALVAL)

HIDE

0:00

<v ->And then finally,

0:01

and I'm only gonna cover this on one slide.

0:05

This entity, adverse local tissue reaction to arthroplasty

0:10

sometimes a fancy name,

0:12

aseptic lymphocytic vasculitis-associated lesion or ALVAL.

0:18

This basically is a category of disease

0:22

in which there is a hypersensitivity reaction

0:25

to what has been used for the joint replacement.

0:29

I see abnormalities most commonly about the hip,

0:33

but also occasionally about the knee and elsewhere.

0:36

And it may be a reaction to metal irons,

0:39

particularly when you're dealing with metal

0:40

on metal (indistinct) or other products

0:44

that can lead to particles that produce this reaction.

0:49

The MR imaging findings that are found

0:52

are masses, they're complex and signal intensity.

0:57

They may be related to capsular defects.

1:01

They may contain fluid, metallic debris

1:05

and thickened synovium.

1:07

And you can see here some examples taken

1:09

from a very nice article by Fritz and others

1:13

in RadioGraphics.

1:15

So be aware of these particular findings

1:18

and the role for MR imaging in their assessment.

1:23

We've reached the end of segment two.

1:25

So again, I'm gonna turn to Marcelo

1:28

to discuss his cases at this point.
