Meniscocapsular Injury with Semimembranosus Tear

Next case is a 38-year-old man with knee injury,

and this is meniscus case six,

scrolling from medial to lateral.

Again, you can see there's all this edema superficial

to the MCL complex.

When we get to the, uh, MCL, you can see

that both the tibial collateral ligament,

which is this structure here

and the posterior oblique ligament,

there should be this triangular shaped structure.

This is the central, um, this is the, the important central,

uh, portion of that, uh, uh, posterior oblique ligament.

And both, both of these are torn at the femoral attachment.

Just you can, uh, clearly see that on the, on the sagittal

as we go deep to those, you can see that there's

a much more obvious meniscocapsular injury than the

prior case I showed.

Uh, there's, there's a lot of irregularity.

Uh, you see the torn fibers in the meniscocapsular meniscal

tibial ligaments with all of his, all of his edema.

The EM knows mo semimembranosus tendon has

increased signal as well.

And this is a partial tear.

It's kind of an intrasubstance tear,

mostly involving the direct, uh, attachment of

that semimembranosus.

And as we go lateral,

you can see the, the PCL uh, looks okay,

but the ACL has a,

um, has a tear.

And as we get to the posterior, uh, ho uh, horn of

that lateral meniscus, uh,

there's some increased signal at this, uh, posterior root of

that, um, it lateral meniscus, but,

but it's not, it's not a high grade tear.

So at, at worst, it'd probably be a low grade tear.

And so this is just another ramp lesion.
