Accessory ITB Meniscal Ligament

0:00

So here's that structure that, uh,

0:02

you saw briefly in the other case.

0:04

This is the so-called accessory iliotibial band

0:07

meniscal ligament.

0:08

It's not seen, uh, too infrequently in knees,

0:12

and if you aren't looking for it, then you have, you,

0:15

you aren't looking hard enough.

0:16

But in this one paper, um, about, uh,

0:19

five years ago now in a JR, they found

0:22

that when you see these, these are associated with some sort

0:25

of meniscal lesion in up to 25% of cases.

0:29

And lesion in quotes could include, uh, a maal meniscus tear

0:34

or intra substance degeneration.

0:36

And they also saw that some

0:38

of these patients also had HFA edema.

0:42

So here's what it looked like in arthroscopy.

0:44

This is a, um, screen grab from that paper.

0:47

And here is a case from our repository.

0:50

Uh, you can see this dark fiber span extending from the deep

0:53

versus surface of the iliotibial band.

0:56

And in this case, you can see

0:58

that there is some intrasubstance globular signal not

1:01

violating the meniscal surface.

1:03

So certainly not meeting morphologic criteria

1:06

for meniscal tear at this time,

1:08

but presumably reflecting, uh, some form

1:10

of intra meniscal degeneration.
