Synovial Plica

0:00

<v ->Okay, we're gonna finish up.

0:02

I know four hours is a long time,

0:05

but hopefully you are still interested.

0:09

You're still awake.

0:10

I don't know what time it is

0:11

for those who are listening to this lecture.

0:16

It is 8:15 here in Cincinnati.

0:19

So it's past my bedtime already as well.

0:23

But we're gonna continue on.

0:24

And we're gonna discuss for a lot of this segment

0:27

the topic of Synovial Plica,

0:31

and these are folds that we find in certain joints,

0:35

to some extent they are normal.

0:37

When they thicken and become irregular,

0:40

they can become abnormal.

0:41

And with that, they can become clinically important.

0:45

It is generally believe that

0:48

their pathogenesis relates to incomplete resorption

0:52

of tissue that normally is present during development

0:56

but tends to disappear or almost disappear in most people.

1:01

We're gonna start the knee because

1:03

clearly if we talk about Plica,

1:05

the most press has been with regard

1:08

to those Plica that occur in the knee

1:10

and I've categorized them into four locations.

1:14

You can see that on the left.

1:16

Now I can tell you that the last of these, the lateral,

1:19

that Plica in the lateral aspect of the knee are infrequent

1:25

and debated as to whether they are a real finding.

1:29

But the other three are pretty well recognized.

1:30

Superior Plica or some people call it

1:33

a super patella Plica

1:35

with a frequency of 30 to 85%.

1:39

In the middle you see a medial Plica

1:41

with a frequency of 30 to 70%.

1:45

And the inferior Plica,

1:46

also known as the ligamentum mucosum

1:49

with a frequency of 20 to 80%.

1:54

Now those are the reported frequency of normal

1:57

Plica, not a Plica syndrome.

1:59

And you can appreciate that

2:02

there's wide variation in what has been reported.
