Retrocalcaneal Bursitis

Now in the normal situation on the radiograph,

there is a radiolucent stripe that passes behind

the top of the calcaneous,

between it and the Achilles tendon.

It relates to a portion of the pre-Achilles fat pad

that extends behind and below the top of the calcaneous.

That lucency has a name.

We call it a retrocalcaneal recess.

It relates to this fat extending down

into the area of the bursa.

What occurs, however, when you have fluid within this bursa,

we have a retrocalcaneal bursitis.

That lucency normally present will be obscured

and a dome-shaped mass of fluid can be appreciated

on the radiograph.

To give you an idea now of what you might see you

with MRI imaging, look at the bottom right image,

you'll see retrocalcaneal bursitis associated

with reactive edema involving the calcaneous.

You also see altered signal

within the Achilles tendon itself,

and a superficial bursitis as well as subcutaneous edema.

This is retrocalcaneal bursitis.
