Lateral Plica: Knee

HIDE

0:00

<v ->The lateral plica and lateral plica syndrome is debated.

0:05

It's rarely reported in the literature.

0:08

It's rarely seen at the time of arthroscopy.

0:11

The people who believe there is a syndrome

0:13

associated with folds in the lateral aspect of the knee,

0:16

say you can have pain, again a snapping sensation,

0:20

and they can feel the plica in any location

0:24

when it's markedly thick.

0:26

I'm taking these images from the literature,

0:28

I don't have any good cases of my own,

0:30

but here's one from RadioGraphics

0:33

showing you a thickened lateral plica.

0:36

Here's another one taken from a Radsource Web Clinic

0:39

showing you what was reported to be

0:41

a thickened lateral plica.

0:44

There's a short medio plica as well, in this case.

0:48

But it brings us to another entity

0:50

that has received some attention in the recent literature.

0:54

So I wanted to say a word or two about this entity.

0:58

This condition has a very, very fancy name, it's known as

1:05

an accessory iliotibial band - meniscal ligament.

1:09

And what it is said to be is a band of tissue

1:13

that extends from a portion of the iliotibial tract

1:16

to attach to the anterior horn and body

1:19

of the lateral meniscus.

1:22

And therefore that lateral meniscus

1:24

does not move in a normal fashion.

1:27

Now, if you look,

1:28

and you have a lot of experience with knee MR,

1:31

you're gonna often see a shadow like this

1:32

but in most cases you won't see fluid on both sides.

1:36

In the article that came out,

1:38

and this came out from Switzerland, in the Zurich area,

1:43

they had beautiful arthroscopy pictures

1:45

showing you this ligament

1:47

associated with problems of a lateral meniscus.

1:50

I'm not sure how specific this is.

1:53

Here's pictures, first of all, from their article

1:56

showing you that particular band or ligament

2:00

attaching to a torn anterior horn and body

2:04

of the lateral meniscus.

2:06

Shortly thereafter, we came across one of our own cases

2:10

that seemed to show a similar phenomenon.

2:12

What appeared to be

2:13

an accessory iliotibial band - meniscal ligament,

2:17

attaching to an abnormal lateral meniscus.

2:20

Some people who believe in this

2:23

and think this is simply a plica,

2:25

not a true ligament, I'm not sure.

2:28

It has been reported in 13% of knee MRIs.

2:32

So to be aware of it

2:33

but I'm not sure it's a proven entity at this point.
