Case: Pulmonary Interstitial Emphysema

So this was a two week old infant who had lung disease

of prematurity and is continuing to be, uh, uh, intubated

with, uh, respiratory support.

Um, this patient does have a left upper extremity pick

peripherally inserted central catheter that extends

to the upper right atrium level.

And he has an tube

with the tip appropriately projecting over the stomach.

So I think this finding is challenging

to tell is it the opacities that are the abnormal finding

or is it the lucencies that are the abnormal finding?

And of course the answer is both

because this patient has lung disease

and prematurity with atelectasis.

Right? We have some increased density in our left retro

cardiac region, but check out these branching, um, kind

of linear and branching lucencies,

not only diffusely throughout the right lung,

but extending through the upper, uh, left upper

and mid lung zones as well on the other side.

So once I see branching lucencies like this,

I'm concerned about a complication

of lung disease and prematurity.

Um, and that's pulmonary interstitial emphysema.

So this patient had a CT for other reasons,

but it nicely illustrates the appearance

of pulmonary interstitial emphysema

and it nicely correlates with that radiograph.

So we see these, uh, branching

lucencies extending diffusely throughout the interstitial

all throughout the right lung.

Um, these loosen lines

and branching, um, lucencies, uh,

parallel the bronchovascular bundles diffusely throughout

the right lung and as well we see them um, in the right, uh,

left, left upper and mid lung zone.

Um, this is a non-contrast chest ct.

Now we're in the axial plane

where you can see these loosened lines

and branching structures going all along the interstem

of the, uh, bronchovascular bundles.

So this is a gorgeous CT example

of pulmonary interstitial emphysema.

A nice correlate to that CT appearance that we saw.

But no pneumothorax, no pneumo, media signum.
