who presented with respiratory distress after birth.

They have placed an enteric tube,

which we see extending into the gastric body.

Um, the important finding here is

that the thymus is uplifted.

So you see this soft tissue density here

with this increased lucency inferiorly,

and this is that angel wing appearance

of pneumo mediastinum.

So gas in the mediastinum when you have a big thymus, which,

uh, infants will have, will cause uplifting of that thymus.

So Pneumo mediastinum will give you this sort

of angel wing appearance of the thymus being lifted up

by this mediastinum.

Um, the rest of the lungs don't look too bad in this infant.

Maybe just a little bit of granularity with a little bit

of superimposed atelectasis.

But the pertinent finding is this Pneumo mediastinum, um,

uh, it's asymmetrically more pronounced on the right in this

infant compared to the left.
