Case: Diverticulitis, Perforated with Abscess

Okay, here we have another CT scan of the abdomen

and pelvis with contrast showing the liver

and spleen to be enhancing homogeneously.

As we come down, we're going to see that the bowels

without evidence of obstruction, the kidneys

look pretty good, but as we come down to that

left lower quadrant again, we start seeing some

inflammation, some thickening along the sigmoid

mesocolon here, and then we actually see.

This is a sigmoid colon with a lot of

diverticula and a lot of diverticulitis,

thickening of the wall, inflammation.

Look at all those beautiful epiploic appendages that

are all outlined in fluid, but adjacent to that

sigmoid colon is a dilated fluid collection and air-fluid

collection and extraluminal fluid collection consistent

with perforation and a drainable fluid collection.

Here we see on coronal again, we have

nice enhancing walls, making that a nice

abscess and target for percutaneous drain.

So this would be a complicated

diverticulitis with abscess formation.

We would look elsewhere in the abdomen

to see if there's any other signs of

inflammation, but this is a pretty classic

appearance of complicated diverticulitis.
