Okay. Another contrast-enhanced CT scan of the

abdomen and pelvis. Coming down, we're going to see

the liver and spleen are looking pretty good.

As we come down into the left lower quadrant, again,

we're going to see multiple diverticulum along the

colon, some inflammatory changes as well. Down

into the pelvis, it even gets more dramatic.

This is diverticulitis, so this is nice diverticulitis.

So here we have diverticulitis, but the next thing

you're going to look for in diverticulitis is

to make sure that you don't have any, you know,

disseminated liver abscesses. You don't here.

But if you look carefully at the portal vein,

you're going to see that you have a thrombosis

within the portal vein, here at the confluence.

You can get IMV thrombophlebitis

in the setting of diverticulitis.

I see it maybe three to five times a year, maybe three,

which is actually quite a bit, and it is definitely

something that you want to diagnose when present,

because these patients need to be anticoagulated,

and this is a higher-level diverticulitis as well.
