You know, there's always been an argument, what causes this?

I hope you all remember

that this is a disorder involving metatarsal heads,

the most common location.

The second, described by Freiberg in 1914

sometimes multiple metatarsal heads were involved,

unilateral or bilateral distribution.

The rarest location was the first metatarsal head.

The exact cause of this was not known.

In fact, it was listed as one of the osteochondrosis,

like Legg-Perthes disease and Osgood-Schlatter disease.

We didn't know exactly what caused it.

What we saw radiographically was flattening

and fragmentation of the metatarsal head.

Later on, intra-articular bodies might be located.

Well, there are multiple causes of Freiberg's infraction.

In an active trial, shown here,

it appears to be a stress fracture.

And if there is osteonecrosis,

it would be a secondary event.

In an elderly woman with osteoporosis,

it may represent an insufficiency fracture

involving the subchondral bone.

And it's a finding that we see in diabetics

perhaps related to a neuropathic fracture

with subsequent disorganization of the joint.

So again, linking together things

that look like osteonecrosis with fatigue fractures,

insufficiency fractures, and neuropathic fractures.
