DOTATATE Case: Pancreatic NET

64-year-old male with

a pancreatic mass that was diagnosed

as a well differentiated neuroendocrine tumor.

DYS disparity was performed for staging.

Looking at the mip, we can see that there are multiple areas

of abnormal tracer uptake in the liver.

There is, uh, very abnormal uptake in the

upper abdomen as well.

And so we're gonna go through all these findings on the

axial plane.

So starting with the pancreatic mass,

there is a large heterogeneous pancreatic mass.

As we can see on the pit,

there is intense peripheral uptake.

The center of the lesion has no tracer uptake.

This mass is centered at the tail of the pancreas,

but is locally aggressive

and extends into the splenic hilum as we can see on the CT

and also in a heterogeneous way on the pit,

as well as extends toward the left

pararenal space and involves the left kidney.

This was the biopsy proven pancreatic neuroendocrine.

It's well differentiated

because we can see that there is intense TA uptake.

Other areas of abnormality are all these multiple

lesions in the liver.

Some are large like this one on the left helo with central

NIA and hypo density on ct.

And some are smaller like this one

on the edge anteriorly,

which has a very subtle correlate on the CT

portion of this study.

As I mentioned earlier, the MIB images allow us to see areas

that may be obscured if we scroll through in axial plane.

And I want to point your attention to this area,

focal uptake that projects anteriorly into the patient's

2:20

body, that it's subtle

and I really have to force the window here to see it better.

These correlated to a soft tissue nodule on the

left anterior abdominal wall.

And so, uh, not only we were able to identify, in this case

the metastatic disease

and the local involvement

of the pancreatic mass into the spleen and kidney,

but we also identified an additional soft tissue metastasis.

We went back to priors and this was a new finding

and this showed enhancement on a CT abdomen

demonstrating that this was not a benign finding,

but a soft tissue metastasis.
