DOTATATE Case: Gastric NET

This is a case of a 57-year-old female

that we have seen previously.

But in this case I'm going to focus on the findings

related to the primary lesion.

She presented with abdominal pain due to polyhis

and was found incidentally a gastric mass

and the biopsy revealed well differentiated

neuroendocrine tumor.

So this study was done for staging.

When we start looking at this case, I'm going

to change the window to be able to identify the areas

of abnormality given that as default as you can see,

the liver has very intense uptake and so does the spleen.

So in order to separate these structures, I'm going

to manually change that contrast.

So now that I have done that, I'm looking at the me images

and I identify that there's an abnormal intense

focal uptake in the region of the upper abdomen,

and the rest seems overall, uh, physiologic,

but we will look at it in better in detail.

So centering ourselves in the abdomen

and scrolling through, we can see that there is these area

of uptake correlating to a partially calcified mass that

arises from the cardio of the stomach.

And this was consistent

with a biopsy proven neuroendocrine tumor.

Now, the key of these study not to identify the primary,

which they knew, but

to address if there is distant disease or novel disease.

In this case, we did not identify any other areas of noal

or disease, so this case was localized to the stomach.

One thing to notice is that there is a focus

of uptake lower down in the left hemi abdomen, that if you

look at it as rounds of tissue,

which is uh, spleen, the same way

that the spleen has uptake the spleen or accessory.

Spleens also take up tracer.
