Pleomorphic Adenoma

HIDE
PrevNext

0:01

Minor salivary gland tissue

0:04

extends throughout the aero digestive system.

0:07

The highest concentration of in the head

0:10

and neck of minor salivary gland tissue is along the hard

0:14

palate and soft palate.

0:15

From there, you can have lesions which may grow into the

0:19

floor of the maxillary sinus from the hard palate, so

0:24

that is where one might see the minor salivary gland plasms.

0:28

The most common of the minor salivary gland plasms

0:31

that are benign is the polymorphic adenoma.

0:34

This happens to be a polymorphic adenoma of the nasal septum

0:39

benign appearance in regard to its rounded shape

0:43

and lack of aggressive borders.

0:46

However, you may also see minor salivary

0:50

gland adenoid cystic carcinomas.

0:53

We'll talk about that when we get to the malignancies.

0:56

Adenoid cystic carcinoma is the most common malignant minor

0:59

salivary gland tumor, and its characteristic feature is a 50

1:04

to 60% rate of perineural spread.

1:07

So we wanna make sure that we look at the branches

1:10

of the fifth granial nerve

1:11

to see whether there's any perineural

1:13

spread in this location.

1:15

As it is associated with the nasal septum.

1:19

We would include the conroy lesion,

1:22

however, the absence of a matrix

1:26

in this particular lesion would favor a different diagnosis.

1:30

In this case, polymorphic adenoma.
