Nasal Septal Perforation

This was a patient who presented with facial pain.

And as you can see on the bone windows,

the patient did have maxillary sinus ethmoid,

sinus sphenoid, sinus inflammation.

But the curious feature in this case was the nasal septum.

Here you see the low density associated

with the cartilaginous nasal septum extending into

the nasal cavity.

And this is the columella of the nasal cavity

projecting into the Aries here,

and you have this low density.

This was an abscess of the

nasal septum in a patient who was a cocaine user.

So this is, I would say like the precursor

or a, a gross precursor to the

nasal septal perforation

that we typically see in cocaine users

where this area of necrosis.

So differential diagnosis here includes pyoderma granulosis,

in which case you have an infection.

Usually I believe it's often pseudomonas infection

that can occur along the nasal septum as well.

But, uh, differential diagnosis would include things like

tuberculosis, like syphilis,

like leprosy affecting the nasal septum.

So this patient had this aspirated

and did have the very ugly purulent material coming out

of the nasal septum in a patient who was a cocaine user in.

As you can see, the inflammatory process extended into the

upper lip and into the adjacent soft tissues.
