Rhinosinusitis Classification

Fungal sinusitis is included in the general

categorization of rhinosinusitis, and I mentioned

And that's usually where the patients

are being evaluated for potential

functional endoscopic sinus surgery.

Acute rhinosinusitis is generally

treated empirically with supportive care,

sometimes with antibiotics when there's a

presumption of bacterial infection as opposed to

viral infection, which accounts

for most of the rhinosinusitis.

So within the rhinosinusitis classification,

we have acute presumed bacterial rhinosinusitis.

Then we have our chronic rhinosinusitis without

polyps, which accounts for 60% to 65% of those

chronic rhinosinusitis.

This is a sinusitis that lasts 12 weeks or more,

and it may have two out of the four: that is, drainage

from the nose, obstruction, facial discomfort, maybe

hyposmia, with a positive CT scan or endoscopy.

Chronic rhinosinusitis with polyps accounts

for 20% to 33% of the chronic rhinosinusitis,

and these are patients who may have

aspirin sensitivity, asthma, some atopic

characterization that leads to the presence

of the polyps and an allergic reaction.

And then within that category, we also have the

classic allergic fungal rhinosinusitis, which I

part of the chronic rhinosinusitis categorization.
