Mucocele in the Sphenoid Sinus

mucocele on this patient's sinonasal MRI.

So here we scroll through, and somewhere

in here, we're gonna find a mucocele.

Now we know that the most common locations are in the

frontal ethmoidal region, but those look pretty good.

The maxillary sinus is an unusual

location for a mucocele.

Maybe some of you might wonder about the sphenoid sinus.

Well, this is the potential pitfall of MRI.

Let me show you the T1-weighted scan.

So here we have a sphenoid sinus, which is completely

opacified with secretions that are slightly hyperintense

on the T1-weighted scan and completely black

on the T2-weighted scan, making it

look like it was an aerated sinus.

So compare this on the T2-weighted with this on the

T1-weighted, and you can see how MRI has that potential

pitfall where secretions may look as black as aeration.

The rest of the paranasal sinuses

are indeed normally aerated.

This expanded sphenoid sinus with

hyperproteinaceous secretions

is a sphenoid sinus mucocele.

Again, not a common location for a

mucocele, but one which we have to consider,

potential involvement of the cavernous sinus.
