Case: Talus Lateral Process Fracture

HIDE
PrevNext

0:00

If this person presents with ankle pain

0:03

after snowboarding injury.

0:05

Here we have three projections of the ankle

0:07

frontals done simulated weightbearing

0:09

because of inability to fully load bear.

0:13

We look at our distal tibia,

0:15

follow the medial malar contour come along.

0:18

The medial gutter mortis is maintained here.

0:22

Lateral gutter, distal fibula.

0:26

Our oblique projection carefully looking at the Taylor Dome.

0:31

We don't appreciate any osteochondral lesions.

0:34

We come to the lateral projection.

0:36

We again identify that tibial Taylor joint.

0:39

The joint space is preserved.

0:41

However, there is an anterior fusion.

0:43

Here we can see displacement of the recess show.

0:47

Part articulation is maintained. Subtalar joint is here.

0:52

And as we look at our angle of josse, not only

0:55

for the alignment of that

0:57

and the integrity of the calcaneus, we now

1:01

identify a little lucency

1:02

through this portion of the tailless here.

1:06

And so now we carefully look at the hind foot.

1:09

We see this lucency through a part of the tailless,

1:12

which represents a fracture.

1:14

So that part of the ALIS is gonna be the lateral process,

1:18

which we'll go back on the frontal to see.

1:20

If we can confirm and we go to our frontal projection,

1:24

we try to identify an abnormality through

1:26

that lateral part of the the tailless.

1:28

But it can be challenging.

1:30

So this patient went on to a CT exam

1:33

and here CT with the multiplanar reformats, we can confirm

1:38

that there is a fracture of the ALI going

1:41

through the lateral process.

1:42

And here there's even a larger

1:44

fragment than suspected on radiograph

1:47

with commuted fragments.

1:49

And so this patient has a lateral process. Tails fracture.
