Synovial Chondrosarcoma

<v ->Now we come to that synovial chondrosarcoma,

0:04

as I say, this is a rare tumor,

0:08

reported in some cases as a complication

0:12

of primary synovial chondromatosis,

0:15

I think this figure of 5% is high,

0:18

but that you can find in some of the literature.

0:21

Seen in the knee and hip more than in other cases.

0:24

And as I'll show you, it looks a little bit more bizarre

0:29

than simply a synovial chondromatosis.

0:34

It can be associated with distant metastasis.

0:39

So here's one example, it's an old case,

0:41

and you look at the plain film

0:43

and there's no way I would call that synovial chondromatosis

0:47

or osteochondromatosis because it looks so bizarre,

0:51

it's so mass-like, it's so irregular,

0:54

it's a calcification, and ossification,

0:57

and this is an old MR spin echo technique,

1:00

but clearly inhomogeneous areas of high and low signal.

1:06

So that's an example of what it might look like.

1:09

And here again, this one sent to me

1:12

by one of our previous visiting scholars,

1:14

is a synovial chondrosarcoma

1:18

again in the temporomandibular joint.

1:21

And that's why I now think of synovial osteochondromatosis

1:29

and synovial chondrosarcomas occurring in this location.

1:33

This one you could diagnose as synovial osteochondromatosis,

1:37

it doesn't look quite as aggressive,

1:39

but histologically this was a chondrosarcoma.
