0:00

Here's all learning objectives for today.

0:02

I'm hope that you're going to be able

0:03

to identify some common Mr guided, uh, breast biopsy

0:07

challenges by the end of this session.

0:08

Be able to modify your procedure

0:10

to overcome these challenges.

0:12

And I'm not gonna give you a little go through, um, of how

0:15

to manually target a lesion in case of software failure.

0:18

Not that software failure ever occurs occur.

0:21

So, um, you know, this is something

0:23

that we see not infrequently in our second Luke ultrasound

0:26

reports in patients who have, um,

0:29

abnormalities on their diagnostic

0:32

MRI, such something like this.

0:34

Lesion two, right breast was not visible

0:35

by ultrasound and Mr.

0:37

Guided biopsy is recommended.

0:39

Well, depending on your attitude

0:41

and your comfort with Emma guided biopsies, you may be,

0:45

or the, uh, honed on or the yay sort of person.

0:49

I actually personally quite like guided biopsies,

0:52

so I'm more of a Kermit here.

0:54

Um, at your institution, you may have one of two setups

0:58

for breast MRI guided biopsies.

1:00

What I'm gonna be doing is going through some

1:02

of the problems and how

1:03

to overcome them rather than straightforward cases.

1:06

So this is a sentinel table.

1:07

It's one of a couple of MRI guided breast, um,

1:10

biopsy tables.

1:11

Um, out there. It's going, allows both scanning

1:14

and biopsies to be performed.

1:16

This whole unit can be wheeled outta the room when a breast

1:20

biopsy is not being performed.

1:22

The patient obviously lies on their belly

1:24

with their head in this kind

1:25

of massage table thing here and their breast through it.

1:28

It's a very nice system. Um, I've used this one.

1:31

I've used another one. Um, and, uh, it's particularly nice

1:35

because it does allow all sizes of women, all sizes

1:37

of breasts, and that there's,

1:39

as you can see under the table, there's very good clearance.

1:42

You can really sort of get under the patient

1:44

and position the breast very well.

1:46

If you don't have a dedicated table,

1:48

then you may have one of these.

1:50

Um, this is, uh, a MR.

1:53

Coil along with the biopsy attachment.

1:56

Um, this particular one is from Siemens

1:58

and this goes on top of the regular MR Table.

2:01

Again, the patient lying on the front of it

2:03

and their breasts going through it.

2:04

It's a little bit less, um, comfortable

2:06

and you can see that it is more limited,

2:08

as are all these table top ones with how, um,

2:12

you can get kind of under the patient

2:14

to pull their breasts down straight.

2:16

And the size of the breasts that can be accommodated here.
