Now let's go back to some labral abnormalities

and talk about some of the clinical manifestations,

talking about causes of abnormalities of the labrum.

Labral degeneration is common with advancing age

and can lead to degenerative tears.

This is not a unique phenomenon.

We see the same thing with the glenoid labrum.

Chronic mechanical alterations can cause problems

with the labrum, okay?

They relate in some cases to acetabular dysplasia

and other forms of alterations In femoral morphology,

trauma can cause lab abnormalities, either major trauma,

for example, a hip dislocation

or minor injuries, twisting injuries, and certain sports.

And these are the ones that have been particularly

emphasized in the literature.

Soccer, hockey, ballet

and golf can cause labral abnormalities.

What was known early on, even

before Mr Imaging came along was the relationship

of developmental dysplasia of the hip

with labral abnormalities, including labral tears

and paralabral ganglia cyst.

I can remember as a resident in radiology,

diagnosing in fact developmental dysplasia with labral tears

and power label gangly insists

because gas in the hip joint would pass

through the labral tear,

and we would see a mass

with gas within it adjacent to the hip.

And we said, yes, we're dealing with labral tear

and a power label ganglia in cyst.

Here with Mr. Imaging more direct visualization

of the pathology that you can see with dysplasia of the hip,

the femoral head sublux laterally, less resting on the, uh,

labrum abnormal, uh, morphology of the acetabulum.

It's not surprising that one could see labral tariff.

With regard to clinical findings, 90 to 95, uh, percent

of persons over the age of 70 years have pathologic evidence

of labral abnormalities,

and many of them have imaging evidence of such abnormalities

as well with regard to pain about the hip,

hip, and groin pain.

20 per to 50% of persons with such pain have label pairs,

and then clicking, which is a common phenomenon

with certain hip pathologies, a hundred percent sensitivity

and specificity for lab abnormalities have been reported.

In some cases. I think that figure is high.

You can have clicking related to other causes,

not just labral pairs.
