So this is a very interesting case.

This is a radiologist

and, um, he had gotten remarried,

he had lost his wife and was all excited about it.

Went on his honeymoon

and, um, he played, interestingly,

I'm not sure I would marry him.

He played two 18 holes of golf over a period

of about 14 hours on his honeymoon.

Um, then the next day he played a single 18 holes of golf

and then he went back to his room and they honeymooned.

Um, they went to sleep.

And when he woke up, he said his arm, uh, felt funny

and he also reported that his arm felt funny

before, uh, he went to sleep that evening.

He was on Eliquis, um, which we didn't know at the time.

We looked at this. And then when he went out

to play golf again, um, not the best way to to

to start a marriage out on the golf course the entire time,

uh, he couldn't grip the golf club.

And, uh, here we see his pronator, Terrys here.

So this is not a friction neuropathy this time,

but we do see the median nerve right here.

And in the lung axis you're gonna see how it

sits in very close proximity and,

and was compressed for a period of time by the head

of his new wife.

So we, we are postulating that he had a small tear, uh,

the wife's head rested on his arm.

He, he didn't want to tell his wife, his new wife,

that her head was too heavy, so he went to sleep

and that's how you get honeymoon paralysis.

And indeed he had it.

Um, and that is a name that is reported in the literature.

let's look at the long axis projection.

You can see this hematoma right here.

And then as you follow the ulnar nerve down right there,

it kind of dives right next to the hematoma.

And I suspect that it was a combination

of the mass effect from the hematoma

and the head of the new spouse that produced a,

a neuropraxia in this case.

Um, so this is my case of Pronator, Terri syndrome,

even though it is not a repetitive friction neuropathy.
