PI-RADS assessment - DWI

0:00

Let's talk about the D-W-I-A-D-C MAP

0:04

scores and what they mean.

0:06

Now these are not PY RAD scores.

0:08

We're just grading DWI

0:11

and I wanna focus on the peripheral zone right now,

0:13

not the transitional zone.

0:16

DWI is the major criteria for the peripheral zone.

0:19

A score of one means it's normal on both the A d, C

0:23

and the high B value DWIA.

0:26

High B value means a B value of 1400

0:29

or above 1500 or 1600.

0:33

A score of two, linear non mass, like WID shaped

0:38

hypo intensity on A D, C and or linear.

0:42

Wed shaped hyper intensity on high B value DWI

0:48

three, right in the middle, focal discreet,

0:52

and different from the background hyperintensity on A, D, C

0:56

and or focal hyperintensity on high B

1:00

value DWI.

1:03

You may have marked hypo intensity on A D, C

1:06

or marked hyperintensity on high B value

1:09

DWI, but not both.

1:11

That's an important distinction.

1:14

A DWI score of four, focal

1:18

markedly hyperintense on a D, C

1:22

and markedly hyperintense on high B value DWI.

1:27

The size is less than 1.5 centimeter in greatest dimension.

1:31

A score of five is exactly the same as a score of four,

1:36

but it's greater than

1:38

or equal to 1.5 centimeter

1:42

in greatest dimension.

1:43

It is preferable to make this measurement on the A DC,

1:48

but if that's not available then on the DWI

1:51

and if that's not available then on the

1:53

T two weighted image.

1:54

In other words, you can't really get a discreet boundary on

1:57

the A DC or the DWI.

1:59

Let's move on, shall we?
