Um, first of all,

before we get to this, uh, first MTP joint

that looks extremely inflamed, notice how all

of the muscles are hyperintense on T two.

Um, that is probably not diffuse infectious myositis.

Um, that is more likely a patient who is diabetic

and has diffuse diabe, diabetic myopathy.

Very common to see kind

of diffuse signal within those muscles.

But what we're showing you here,

and of course that's a setup for this patient,

placed spas 'em at risk.

That nice smooth joint surface of the first MTP joint, uh,

has been kind of destroyed very irregular.

There's subluxation of the joint.

We have marked hyper enhancement of the bone marrow

on both sides of the joint.

The joint is destroyed.

And so this being, um, a case of infection

and septic joint, um, of the first toe, um, you see,

um, terrible destruction of the bone surfaces,

hypo 10 signal on t2, not a normal, uh, anatomy

that we're being able to find.

And so this was, uh, severe infection there.
