This next video. I'm going to show you how to use ombra
0:03
to aviate coronary CT examinations. Ombra
0:06
has the ability to do some
0:09
reformatting, which we're going to talk about on the next
0:12
video on how to read coronary CT.
0:16
I'm going to demonstrate how you do that reformatting here on
0:19
ombra for the subsequent cases. I'm going to be using my
0:22
own packs workstation just to
0:25
make it a little smoother.
0:27
Let's start just with with how you approach this on ombra. So
0:30
first by default, the tool that
0:33
you need is actually not present on the
0:36
Ombre toolbar. So you want to start by hitting settings.
0:39
And then go into the toolbar on the left hand side. And
0:42
then on the available tools on the left, you want
0:45
to grab the one that says NPR so you have to scroll down to
0:48
find it under Advanced visualization and you left
0:51
click and drag into this middle column, which says toolbar and
0:54
it doesn't really matter where you place it. Once you do that
0:57
you hit save and then that puts the toolbar up on
1:00
the top for you right here. Now, the
1:03
one thing that's not ideal about that is that then it
1:06
has to reload all the images again for this
1:09
case.
1:10
So we'll have to wait just a second as that happens. And what
1:13
you want to do is grab the
1:16
thin slice images and so you'll notice that we have a scout image
1:19
here calcium score image.
1:21
Just fit the images here. That's actually some thick slice
1:24
images. And then this 300 image stack, which is the one that
1:27
we want to use to look at the coronary CT.
1:31
So I'm going to drag that over into the main window.
1:34
And then you can see that I can scroll up and down and assess the
1:37
coronaries on the axial images.
1:40
I can certainly use the window level feature
1:43
by clicking the level button up here.
1:46
And then going back to scroll to make sure I get the level
1:49
set correctly.
1:51
The thing that becomes important though in coronary CT and
1:54
we're going to see that as we go through some of these cases on the subsequent
1:57
videos is that it is important to be able to make
2:00
your own planes using the multiplayer reformat tool
2:03
in order to get the best visualization of the coronary
2:06
that you can so I'm gonna show you how to
2:09
do that on hombre.
2:11
So we start with the NPR tool
2:14
up here.
2:17
And then once you click it, you'll notice this pop-up that says
2:20
sync window levels which you once selected and then
2:23
allow rotations. You want to make sure you trigger that on.
2:26
And then once that's on you'll notice there's a little gear here on the
2:29
bottom left image.
2:31
Which says resent your reset last rotation reset all
2:34
rotations, so I'm going to put that to the side because we're going to use that.
2:37
And then you'll see that we have.
2:40
Different colors there's a purple line an orange
2:43
line and a green line and they correspond to the different Windows here.
2:46
You can see there's an axle which is the orange the purple
2:49
is the sagittal and the bluish greenishes
2:52
the coronal
2:54
so if you scroll down if I wanted to make a plane to
2:57
get
2:58
Nice visualization on the right corner artery. What I
3:01
can do is I can grab the crosshairs drag it
3:04
over. So I'm moving my planes down here and then
3:07
I can actually left click and drag and rotate the plane
3:10
into the
3:12
Navy Groove head. This is the way the standard way to get
3:15
a nice look at the right corner artery.
3:18
And then what I want to do is I want to go to the sagittal image scroll a
3:21
little bit and then tilt my plane forward like
3:24
that so that I'm lined up.
3:26
With the top and the bottom of the right corner
3:29
artery and then once you do all that, you can actually double click here
3:32
and let me just adjust the window a little
3:35
bit.
3:36
and you get
3:38
nice visualization of the right corner artery in
3:41
the AV Groove. This is a nice way
3:44
to look at the long axis of the corner arteries.
3:48
It's really important to be able to manually evaluate
3:51
the corner arteries using these data sets
3:54
and mprs because the tools that we use
3:57
the cprs that you're going to see me use a lot in this course,
4:00
they don't always work 100% And
4:03
that's because CPR success
4:06
depends a lot on the quality of the study and you
4:09
know, although the studies in this course are going to be all diagnostic quality.
4:12
Sometimes you get some some studies which are more
4:15
iffy and those cases then you're going to rely more heavily
4:18
on manual and PRS. So it's good
4:21
to practice them with ombre. Once you've done a couple reformats.
4:24
Actually. There's one sort of
4:27
kink in the system, which is that you have to in order to move the planes again.
4:30
You have to reset the plane. So in order
4:33
to do that, you hit this little gear
4:35
And then you can hit reset all rotations.
4:38
And then you can start over.
4:40
So that's the the best way to manage this situation when you
4:43
need to do some other reformats, you reset
4:46
the planes and in general the concept for
4:49
using the nprs is that if you want to
4:52
evaluate the vessel and evaluate a stenosis you
4:55
drop the crosshairs on this stenosis
4:58
of Interest Let's Pretend There's a stenosis right here.
5:03
In this case. I'm going to reset then you want to
5:06
drag the plane so that one of your planes is
5:09
aligned with the long axis of
5:12
the vessel.
5:14
You drag the other planes so that the other plane here is lined
5:17
with up with a long axis of the vessel.
5:20
And now you have three views you have two long axis views
5:23
and a short axis View and this is honestly the best
5:26
way to evaluate for stenosis and you can zoom in
5:29
if you want.
5:32
If you have scroll if you want on the
5:35
short axis view, you can grab the plane and
5:38
actually rotate it all the way around.
5:41
And while you're doing that you can watch what happens
5:44
to the vessel in the upper right hand corner.
5:47
And find where you see this stenosis is best visualized
5:50
and you can do the best assessment of stenosis severity and
5:53
we're gonna get into this a lot more in subsequent cases,
5:56
but I just want to show you this how to approach cases with
5:59
the Ombre software and you'll have access
6:02
to all the cases that we go through in the subsequent videos via ombre.
6:06
So thanks a lot. That's helpful.
