This is an MRI of the abdomen and pelvis in a

0:03

patient with a known twin gestation presenting for

0:06

further evaluation for operative planning.

0:09

We have steady state free procession images on the left and

0:12

turbo spin echo images on the right.

0:15

As we scroll in Fairly we see the gravid uterus come into

0:18

view with T2 heterogeneous. Myometrium in

0:21

the periphery of the uterus. We see the placenta for

0:24

the more severely located fetus.

0:27

As we scroll inferiorly. We see a

0:30

second gestation with a much smaller fetus present. This

0:33

fetus was selectively reduced earlier on in

0:36

the pregnancy due to its location.

0:39

These are sagittal T2 weighted Mr. Images for

0:42

the same patient again, we see a superiorly located

0:45

fetus and amniotic sac with a

0:48

fundal and anterior placenta. Both of which are relatively normal

0:51

in appearance.

0:52

On the sagittal view we're able to more clearly see

0:55

this thin band of intervening myometrial tissue

0:58

between the two gestational sacs.

1:01

We see that the more inferiorly located gestational Sac

1:04

with the smaller fetus is centered in

1:07

the low anterior uterine segment.

1:10

Additionally we can see that there's marked heterogeneity of

1:13

the paraservical tissues and particularly

1:16

the posterior lip of the cervix which has

1:19

numerous T2 dark flow voids present internally.

1:24

These are coronal T2 weighted images of the abdomen and pelvis in the

1:27

same patient steady state free procession images on the

1:30

left and turbo spin echo images on the right.

1:33

Again, we see the superiorly located intrauterine gestation

1:36

with a fundal placenta and a

1:39

second trimester fetus.

1:41

We can nicely visualize this thin band of intervening Miami

1:44

trim between the two gestational sacs

1:47

again with a small fetus. I want to

1:50

point out the heterogeneity of the tissue surrounding the

1:53

smaller fetus and highlight the fact

1:56

that it this is really early chorionic proliferation

2:00

and early placentation.

2:03

Revisiting the sagittal images we can

2:06

again see that the smaller fetus and smaller

2:09

gestational Sac contribute to a mild amount of

2:12

bulging and thinning of the low anterior uterine segment

2:15

in addition to that vascular proliferation in

2:18

the region of the cervix. And so the diagnosis for

2:21

this case is a coexisting intrauterine gestation

2:24

with a cesarean section scaric topic

2:27

pregnancy.
