Hello everyone, Dr. Sydney Levy here continuing our discussion
0:03
3 00:00:05,130 --> 00:00:08,490 of the anatomy of the hypopharynx as a
0:08
prelude to the diagnosis and staging of
0:11
hypopharyngeal squamous cell malignancy.
0:14
I'd like to focus on the piriform sinus.
0:17
This is one of the three subsites of the
0:20
hypopharynx, the other two being the post-cricoid
0:23
space and the posterior hypopharyngeal wall.
0:27
The piriform sinus is the most commonly involved
0:30
subsite of hypopharyngeal squamous cell malignancy.
0:34
And I'd like to draw its boundaries for you
0:37
and explain some of its anatomical relations.
0:41
I have pre-contrast non-fat-suppressed T1-weighted
0:44
imaging in the axial and coronal projections to assist.
0:49
So firstly, let's draw some boundaries.
0:53
On the axial projection, the piriform sinus
0:58
begins at the junction of the medial and
1:01
lateral walls of the aryepiglottic fold here.
1:05
From there, we follow the lateral wall to the
1:11
piriform sinus itself all the way around until
1:16
we reach the same level at which time we cross
1:19
over into the posterior hypopharyngeal wall over
1:25
to the other side where the situation repeats.
1:29
On the coronal projection, we have the epiglottis
1:34
here, and there is a soft tissue fold which connects
1:38
the epiglottis to the hypopharynx, or the piriform
1:42
sinus, which we call the pharyngoepiglottic fold.
1:46
That's actually here, and from there
1:49
we get into the piriform sinus.
1:54
This is the lateral wall
1:57
of the piriform sinus.
1:59
There is the apex.
2:01
And then we head back up to the level at which
2:06
the piriform sinus meets the aryepiglottic fold.
2:10
Now this here is the aryepiglottic fold.
2:14
So this is a little bit difficult to picture,
2:16
but here we have the pharyngoepiglottic fold.
2:22
Here we have the aryepiglottic fold.
2:26
And remember that the lateral wall of
2:29
the aryepiglottic fold forms part of the
2:33
piriform sinus as a space, as a subsite.
2:37
So, summarizing that, the piriform sinus extends
2:42
from the pharyngoepiglottic fold superiorly,
2:45
as far as the junction of the medial and
2:49
lateral walls of the aryepiglottic fold.
2:53
And this also happens to be at the level
2:56
of the superior margin of the post-cricoid
2:59
space, which is at that level, but medially.
3:04
Other regional structures which are worth
3:06
keeping in mind when looking at this region are
3:09
the medial relations within the supraglottic
3:13
larynx, which include the arytenoid cartilages
3:16
here, a little bit further down, and the thyroid
3:21
cartilage, which is this large structure here.
3:25
So in summary, think of the piriform sinus as
3:29
consisting of an apex at the bottom, anterior,
3:33
lateral, and posterior walls. And keep in mind
3:38
that it is situated adjacent to the supraglottic
3:43
larynx at the level of the aryepiglottic folds,
3:46
the epiglottis superiorly, to which it communicates
3:50
via the pharyngoepiglottic fold, and that it is
3:53
continuous with the posterior hypopharyngeal wall,
3:58
medially and posteriorly, and that the post-cricoid
4:02
space is adjacent to its medial margin anteriorly.
