So in these patients it can be very difficult

Um, particularly in young patients,

I found this to be a challenge.

So what can you do? You do have

to apply more force than you may be comfortable with.

But when you're applying that force,

really look at your hands

and look at the image to see the direction

that you are applying that force.

Is it gonna be in a safe direction?

So if you overshoot a little bit during the insertion,

is it all gonna be fine, still well within breast tissue

or might you be going towards the chest wall?

And it's a little unnerving sometimes to do these, so

it can be quite uncomfortable to patients, um, as well

as unnerving to them if they see that you're kind

of really struggling to shove that needle in.

So there are some modified techniques I'm gonna show you,

um, such as what I call, shoot and slide.

Now you really can improve the safety when you're trying to,

you know, push a little harder than average

with these techniques by modifying your hand grip.

This is not a safe hand grip.

But it's the same problem when you've got your hand over,

you have much more freedom of movement to be able to,

you know, if it's suddenly the resistance goes to be able

to go into far.

So I would highly recommend that for all biopsies.

Um, you use this underhand grip

or more of a pen grip, um, to go in

because then the side

of your hand is resting against the patient

and you really have very little freedom of movement in terms

of wrist rotation as to how far you might overlo

in these patients with dense tissue or mobile lesions.

You can also use the side of your hand

So I developed a technique that I call, shoot and slide

Uh, you do need an introducer to be able to do this.

So what I'm gonna do is I will inter, uh,

put the introducer in as far as I feel that I can, um,

safely do without undue force.

And you can see here I am a, a significant distance, um,

with the tip here from my lesion here.

I'm then gonna put the needle in, I'll deploy the needle

directed towards the lesion, realizing

that it might not reach the lesion as it hasn't.

Here you can see that the, uh, tip is actually, um,

The important thing is then step three,

which is you advance the introducer over the needle.

So you're kind of using it like a coaxial technique.

And I find what I do usually is I just rotate

that introducer, um, push it forward

and it's gonna slide beautifully over the needle with no,

you know, no resistance whatsoever.

You then remove the needle, take your sample out,

I toss it in the pot, you know, knowing

that probably hasn't even got the lesion in it.

And then when you put the needle back in,

your introducer is now in the perfect position

and your second throw is gonna

go straight through the lesion.
