Circumscribed Mass – Fibroadenoma

0:01

Okay, so this is another companion case and also

0:03

a good case to transition to our next section.

0:07

This is another baseline screening mammogram,

0:10

and this patient has an oval circumscribed mass in

0:13

the upper outer right breast, which we see here.

0:18

Again, like the last few cases,

0:20

this was given a BI-RADS 0.

0:22

She came back for additional mammographic images,

0:24

and those are usually the spot compression images.

0:27

This finding persisted on the spot compression

0:29

images, and so we did a targeted ultrasound.

0:34

Here's the images from the targeted ultrasound.

0:38

We see this oval circumscribed mass.

0:42

Now, this is not a simple cyst like we saw

0:44

previously, and it's also not an intramammary

0:47

lymph node, which we also saw previously.

0:50

This looks like it's probably solid.

0:52

It doesn't have typical

0:54

appearance of a complicated cyst.

0:56

For example, we don't see through

0:57

transmission or an echogenic back wall.

1:02

This finding meets criteria

1:03

for a BI-RADS 3 on ultrasound.

1:06

BI-RADS 3 on ultrasound, the first finding

1:09

that we'll talk about is an oval circumscribed

1:12

mass, so similar to the mammographic finding.

1:16

So this finding actually meets criteria for

1:17

BI-RADS 3 on both mammogram and ultrasound.

1:21

Because this finding correlates, this is

1:24

the same finding on the two modalities.

1:26

We can just do ultrasound to follow it, so we don't

1:29

have to expose the patient to extra radiation,

1:32

even though it meets criteria on both modalities.

1:35

So an oval circumscribed mass is also an

1:38

appropriate BI-RADS 3 finding on ultrasound exam.
