Large Bowel Colonic Obstruction

Okay, so here is our patient with a

high grade colonic obstruction.

This patient actually has a history of prior

colon cancer that is status post-radiation, and

that was a radiation stricture in the pelvis.

But notice how dilated the colon is.

It is extremely dilated. Distally,

we have that very narrowed obstructing region of

radiation fibrosis, and this is a problem, right?

So the question I have for you is,

is this a closed loop obstruction?

The answer is yes.

All right.

Because the distal obstruction is at the

level of the stricture, of course, but

proximally, you have the ileocecal valve.

Frequently you can see the ileocecal valve

because they just have these little,

we call them fatty lips of the ileocecal valve.

That's where the terminal ileum comes into the secum.

But that can work as a one-way valve,

or I like to say a have a heart trap.

This is a little trap you can get if

you have a mouse infestation where the

mice can go in, but they can't come out.

That's a similar principle of the

ileocecal valve, the feces can get

through it, but can't come back.

So as a result, if you have a high grade

colonic obstruction, it is indeed a closed

loop obstruction because the contents of the

colon can't go pro-grade through the colon,

and they can't be drained retrograde either.

So this is actually an emergent situation, not

an urgent, not, um, you know, maybe tomorrow

morning this is a situation which is emergent.

You should definitely raise all of the flags

to your referring clinicians to know that

this is under tension and that there could

be a high likelihood of perforation here.
