Post Lumpectomy - Benign Fat Necrosis

This is a 78-year-old female with a history of prior

lumpectomy or cancer treatment in the right breast.

She's presenting for a screening, uh, mammogram.

The left breast is negative, uh, in this case,

demonstrating just some coarse calcifications.

On the right, we see the stereotypical

appearance, postoperative findings.

We see multiple surgical clips.

You see a surgical scar marker, uh, in the breast.

In this case, the periareolar incision was

done, and then also in the right areola,

probably related to her lymph node biopsy.

At the surgical site, we can see this large

heterogeneous calcification, which has more

of a rim, uh, sort of configuration, right?

Some denser at the periphery,

as compared to the center.

Um, and this finding is compatible with...

As we look at, uh, the DBT images, they don't offer

us a whole lot more of information in this particular

situation, but we can get a better sense of the

appearance of that, uh, area of fat necrosis, right?

It closely, uh, corresponds to probably a...

previous seroma, uh, cavity potentially.

Um, and we see those dense, uh, peripherally

located, uh, calcifications.

At that site, we see similar findings on the MLO view.

Again, rim calcifications in this

large area at the surgical site.

Taken all that together,

this can be considered benign and

just recommend routine screening.

In some cases, fat necrosis at the surgical site,

when it's in its earliest phases, it's developing,

the calcifications, uh, can sort of present as

small calcifications with differing, uh, sizes

and shapes, which is the same, uh, way we would

describe fine, say, amorphous calcifications.

So it's not uncommon, uh, to have a patient recalled

from a screening exam, uh, shortly after her

surgery, uh, in the evaluation of

calcifications at the surgical site.

Um, and occasionally those patients do, on subsequent

biopsy, but in the majority of the cases, uh, the

calcifications are related to benign fat necrosis.
