Gynecomastia - Marked Dendritic

0:00

This is a 61-year-old, uh, male who's

0:03

presenting with a palpable finding in the

0:06

left breast. For male exams, we'd start with

0:11

standard 2D full-field digital mammography.

0:14

On the left side, we can see that there is,

0:16

um, a subareolar, uh, focal asymmetry, which

0:22

looks like normal fibroglandular tissue

0:24

that we would see in a female breast.

0:27

And this likely corresponds

0:29

to his, uh, palpable finding.

0:32

We can see that this has a pattern of sort of

0:34

a flame-shaped appearance with interspersed

0:37

fat, looking like normal range of the tissue.

0:39

This would be considered the dendritic pattern

0:41

gynecomastia, and in this case, uh, I would

0:44

consider this at least moderate or marked.

0:47

Um, on the left side, asymmetric from the

0:50

right where he has just a small amount of

0:53

glandular tissue, uh, in the subareolar location.

0:56

This patient did end up having, uh, some

0:58

additional spot compression views on the left.

1:02

Um, but I'll show you, um, just the full-field

1:06

and this was likely done, uh, on account

1:08

of the palpable finding that he identified

1:11

was at a location that was remote from the NL.

1:15

Most of the time, male patients

1:17

present with a palpable finding, which is

1:19

beneath or directly below the nipple.

1:22

In this case, it was a little bit different

1:23

as he felt this finding a little bit, um,

1:25

further lateral from the nipple. But scrolling

1:29

through the tomosynthesis slices, we see that

1:31

this looks like normal fibroglandular tissue,

1:34

and the patient can be reassured that there, uh,

1:37

is no abnormality other than his normal mast.
